Hello everyone,
These are osstem bone level implants which i placed 3 months ago. Upper right first premolar was immediate after extraction with grafting in the gap. 3 months after you can see some slight bone loss especially mesially of the upper premolar implant, would like to know the cause and how to prevent this? Also if implants are inserted with low torque (spinners) what do you recommend in these cases

For the upper pre-molar, potentially, it may not have been deep enough. The x-ray does look like it could’ve been deep enough, but if there was some buccal or Palatal bone loss on the mesial then it may had to have been slightly deeper to compensate for that and if there’s too much bone loss on either of those walls, it would definitely be more predictable just to graft and come back and place a heel site implant.
On the lower healed site, implant placement if you have an implant a very low torque of five or 10 n/cm or so do you want to make sure you under prepare those sites and if there is sufficient Bone with then you can remove the implant and go up one or two sizes to get more mechanical stability at time of placement

Thank you very much, very well explained👌. In terms of the low torque would you slightly wait longer for opening up and placing healing abutment? Implant was placed 3 months ago, would you wait till the 4 month/ 4 and a half month mark?

Typically 4 to 5 months is more than enough time for osseo integration to take place

Another thing to consider is the proximity to the tooth anterior tooth. Did the anterior teeth have any perio-disease? Studies show that disease from neighboring teeth can extend up to 2mm outwards. This might be something to consider. Thanks for sharing!
