      Quite a challenging case here, what would you do for this lower anterior sector. The two lower central incisors were extracted 6 months ago and the area has healed but still vertical bone loss is observed. Here is a video of the cbct scan.

      Options to do:

      – extractions of lower laterals followed by vertical and horizontal bone grafting with xenograft and collagen membrane taking advantage of bone peaks in canines. Implant placement 6 months after in positions of lateral incisors and 4 unit bridge.

      – extractions of lateral incisors followed by immediate implants in laterals and bone grafting where the centrals are. Taking into account crowns will be huge due to low positions of the implants (crown to root ratio will be affected)

        • This is the case where I typically would remove all four lower sizes and do a ridge augmentation. Wait five months take a CBCT and the patient healed well place to implants for implant bridge. In some cases where there is very little to work with sometimes they might need an additional ridge augmentation at time of placement.

        • Also, I’m not worried about Crown to root ratio. It does not have the same effect with implants.
