        Hello everyone,

        I have a patient with a fractured Straumann tissue-level implant at site #31. The coronal portion of the implant has fractured off circumferentially, leaving only the bone-level portion intact. This implant previously supported a gold crown on a one-piece abutment. Additionally, the tissue-level implant at site #30 has a worn and fractured crown that needs replacement.

        Given the patient’s age and the challenges associated with explantation, we need to explore creative solutions to restore this implant without removal. I will provide additional radiographs and images next week for further assessment, but I’d appreciate any outside-the-box ideas in the meantime.

            let me also clarify the PA. that is a Closed tray impression coping screwed on to the fractured implant.

                I would love to see what the remaining platform looks like. It may surprise us!

                If the remaining piece is fractured in a way that an abutment would leave some irregular gaps and holes, you may consider using your impression coping to try to capture the shape of the remaining fractured implant. You can do that by applying light body impression material at the base of the impression coping before you seat it. Then, squish it down into the implant and let is set up.

                You will want to carefully apply some vaseline into the implant with a microbrush first so that you don’t get the impression material stuck inside.

                Tough case man! See if you can take a picture of the remaining fractured platform! It may alter the course of treatment here.

                  Heres some additional imaging and info. #30 is also straumann tissue level crown that needs replacing. a healing abutment was placed on #31. you can see a void where the platform broke off. the abutment is only secured by the end of the abutment screw.

                  our initial plan is to make new crowns #31/30 and splint. hopefully a custom cast abutment #31 if we can find a lab to do it.
