      CUSTOM HEALING ABUTMENTS

      Hey folks! Hope everyone had a wonderful thanksgiving!

      I went down a custom healing abutment rabbit hole yesterday and wound up with a few lingering questions.

      The protocol for immediate implants makes total sense.. but how can we make custom healing abutments for delayed implant placement?? Does anyone do this and have a protocol they can share?

      Are any of you using this file to print healing abutments? Do you modify the file based on your case before printing? Or do you just have them ready to go and modify them chairside?

      • Hey @RYAN Thanks for sharing this man. You got me going down the rabbit hole as well.

        I’ve periodically make them to use on the day of implant placement. Not so frequently as a stage 2 procedure.

        A couple things:

        The product you shared led me to several instructional videos that tried to teach me to use some free new software. The truth is, i think people would rather not have to learn a new software for this–HOWEVER, i found that Baron Grutter also offers the ready made stl files. I’m going to purchase and print these right away.

        The Cervico kit was very popular for some time, and they have introduced a more affordable option now as well. Probably worth checking out: https://www.vpicervico.com/cervico-products/p/cervico-essential-mold

        I also have been very interested lately in Anatotemp. Its basically the same thing except it is scannable to take an impression. I think it’s super cool. Given that only a handful of implant systems have this functionality currently, i think this workflow is going to be blowing up very soon.

        • @ivan-chicchon I would love the icervico kit but not for $1500 😅😅

          I think the cool thing about learning this software would be customizing it by case, but I’m not sure how feasible that would be

          Let me know how you like the pre-made stl file!

          I was thinking maybe I could print the pre-made stl and then create an icervico like silicone mould to easily make these chairside in an office where I don’t have a printer

          • @RYAN it would be great to eliminate the need for a tibase. The tibase adds cost and add a step.

            • @RYAN im familiar with it but haven’t used it. Might be worth just purchasing all of these options and messing with them.

              @ivan-chicchon I am JUST STARTING my journey with custom abutments. my first case involved taking a Dess Temp abutment and adding composite and chopping off the coronal portion.

              it’s definitely an area I want to level up and provide my restorative doctors with a much better tissue and emergence profile.

              • @mickyfrick I’ve seen that process done quite a bit. Seems like there has to be an easier / quicker way 😁

                • @mickyfrick that’s basically what I’ve been doing up until now. It’s a much easier with PEEK abutments. But I’m in the same boat, gonna look to level this up for myself asap

              • I know iPhysio is coming soon to the US. It looks very promising.

                “The iphysio® Profile Designer is screwed into the implant during the 1st or the 2nd surgical intervention. It remains in place throughout the bone and gingival healing process as well as during the taking of an impression irrespective of the implant sector, whether aesthetic or not. It is only removed once to place and finally screw in the final restoration.”

                • @87madrigal that’s really intriguing! Do you know if these are “stock” healing abutments or are they custom ordered per case?

                  • @RYAN I believe they come in as a kit and have different shapes “custom” so for posteriors you will have a few sizes and for anteriors as well. We’ll see if they are able to bring them to the US.

                • No brother, sorry but I’m not gonna be able to give you an input here because I may be not so old but I’m definitely old school and I use pre-made healing abutments that I get from the implant company

                    I just bought the cervico kit based on this conversation I bought that STL file from Gruter as well. I feel like neither of them will be the ultimate solution, but I want to experiment with them. @mickyfrick @RYAN @87madrigal

                      one thing I dont like about the DESS temp abutment is that that the cervical portion of the abutment (right by the implant abutment junction) is so wide, I have to shave off bone just to get it to seat all the way down. are there abutments out there that are more gradual with the taper?

                    • One technique I use for posterior teeth is scan the implant position with a scan body at the time of implant placement. Have a custom abutment or temp crown fabricated and place it at stage two uncovery. It saves a ton of time compared to fabricating a custom healing abut chairside.

                      • @drcajee great idea! How do you design the healing abutment?

                        • @RYAN You basically have a crown designed to ideal emergence And instruct the lab to cut off what would be exposed above the gingiva. Then at stage two you uncover the implant and close the tissue around it.
