      Hoping for some feedback here as I am fairly new in my implant journey and have had a couple failures lately that I cannot understand.

      Placed this implant a few weeks ago on healthy patient site #30. Surgery went very smooth, with sequential osteotomies and implant torquing out at 40N/cm. Patient comes back three weeks later and implant is completely spinning with large infection. Brand new set of burs (maybe my second set of osteotomies with that kit) and plenty of water to prevent overheating. Anything I should be thinking about or considering when looking at why these are failing?

      This particular patient I did not prescribe an antibiotic. Can that really be the reason this failed so quickly?

      Any feedback is appreciated.

      **Disclaimer: I have discussed apex of #29 with patient and he is aware of site. Also, I ideally would have seated the implant another 1mm sub-crestal if able to re-do this case. (Oh wait, I will re-do this case!) **

      • EDIT: Patient returned today to have implant removed. Removal went fine. But the bigger point is that we found out the issue in this case. Unknown to me, patient is a heavy bruxer had been wearing an orthodontic retainer that was applying significant vertical and lateral force to the implant healing cap. Obviously, this lateral force cause implant instability and most likely is the reason for failure. Frustrating this happened, but happy to know it had nothing to do with my surgical technique!
