Hey whats everyone’s opinion on using collagen membranes after socket preservation where the site heals via 2ndary intent?

I had a recent bad experience where a ptfe membrane kept slipping out. I retucked the membrane and resutured, but it popped out again. Should I have tucked a longer membrane? The patient was wearing a flipper (#8) and may have loosened it.

Is there a tool or technique to prevent the membrane from folding over itself

Ive been reading the Pikos bone grafting book and he shows examples of just using stuff like oraplug or colla plug in the extraction sockets. He then uses a biopsy punch of the palate and sutures it on top. So basically bone graft then a small piece ( uses the biopsy punch) of collaplug then biopsy punch again of keratinized palatal tissue and sutures. I like PTFE membranes like mentioned above they seem to work very well when I cannot get primary closure.

@franco i like this technique that you’re describing. I feel like if you get into the habit of doing it this way, youd get really nice results. I always mean to do this, but then i realize that i didn’t tell the patient ahead of time that i would be punching into their palate and then i ditch the idea

How often does the punched-out tissue necrose?

Sorry I missed this message man. I don’t do it this way often enough to give a good answer. I feel like as long as you leave thick enough tissue intact you shouldn’t have too much trouble with necrosis
