Hey guys just wanted to get to know your thoughts on what I can do to improve. This is a 78 yr old patient with controlled diabetes/hypothyroid/high BP. She takes her medications regularly and reports to be controlled. #29 was extracted due to fracture in October 2022 with bone graft. Implant Astra Osseospeed EV size 3.6×9 was placed in March 2023. She came back in earlier this month with a failed implant.
The PAs below are of #29 before extraction, angle verification, immediate post-op, 1 week post op and the most recent PA. I also included the CBCT slices.
Torque was 40+Ncm.

I created a deeper osteotomy today and placed a larger longer implant after removing the failed implant and thoroughly curetting and irrigating with CHX. Any guidance and feedback is welcome.

I’ll look into her A1C. When I was drilling into native bone, today, I had a really hard time pushing the pilot drill to the right depth. That is at the most common cause of overheated bone? I finally had to increase the motor rpms to 2000 to get the pilot drill to bite into the bone.
