    Good day everyone, I have a case which may involve challenging sinus lifting at the area with sinus septum. LIterature has shown that the risk of perforating sinus lining is low as long as the drilling does not exceed 2mm over the sinus floor or wall.

    In this case, should I avoid sinus lifting and engage the implant fixture right at the sinus wall bony cortex? Can I go 1 or 2 mm beyond the cortex? Based on your experience, does this affect the longevity of the implant and also cause inflammation of sinus membrane? The planned implant diameter is 4.1x8mm.

    In this case, it looks like a minimal sinus augmentation is needed. There is a technique you can consider for this clinical scenario called a ‘sinus implosion’. For this technique, you drill your osteotomy just shy of the sinus floor cortex and then you place your implant fracturing the sinus floor into the sinus. So it often looks like drill 6 mm and you place an 8 mm Implant.
