    Hi,

    I’m planning for #30 implant (5×8.5mm virtually planned here). My concern is there is a vertical bone deficiency (about 3.62mm depression compared to the adjacent bone peaks) and that without vertical augmentation, the implant appears to be too submerged…which also appears to lead to a greater than ideal distance from the gingiva to the top of the implant platform from 5.7- 6.2mm (ideally I would like this distance to be 4mm…5mm at the maximum). I have not performed any vertical augmentation procedures but do have some knowledge of what to do if needed.

    Is it acceptable (good long term prognosis?) to place the implant in this position without vertical augmentation? What would you do here in this situation? TIA.

    When I look at this case, you have enough bone to secure your implant. Regarding the vertical deficiency, I wouldn’t be too concerned as long as the Implant prosthesis has enough tissue support. If it doesn’t, I would consider a connective tissue graft and custom healing abutment.

    I agree. You should be good if the implant has enough bone support. However, I would mention the deficient gum support to the patient and show him/her with visuals, then document in your notes that options were given about potential outcomes down the way.
