<p>Yes it is my first. I have placed lots of single unit implants, but this will be my first overdenture case. For the mandible, we are restoring with a partial denture. </p>

<p>#20 is planned for an extraction and socket grafting.  I planned for an implant 4.0x7mm (which I&#8217;m thinking is a bit short &#8211; would have liked at least 8.5mm) but is limited in length due to the lingual concavity.  </p>
<p>The implant fits into this pre-extraction dimensions; however, I&#8217;m anticipating some bone loss even after socket grafting and&hellip;</p>

<p>As you can see over here the anatomy of the sinus is irregular as the residual bone height is a lot higher buccally and goes down slightly palatally</p>
<p>In this case would you rather:</p>
<p>1. Do a crestal sinus left approach and place a 4.6(wide diameter)x7.5mm implant. Increasing the diameter to compensate the height in that case&hellip;</p>
