      Hi all, Im trying to place a single unit #13. Due to boneloss, it looks like the buccal side is longer than the palatal side. If I were to place #13 completely submerged, 1mm subcrestal on the lingual side, it looks like it would be 2.2mm subcrestal on the buccal side. Should I shave down 1mm of the buccal bone to make it more even?

      If so, do you typically charge this? What code do you use?

      • I would not shave down anything. If needed at time of uncover you can remove any bone around the cover screw as needed. That being said I always shoot for 2mm subcrestal to create enough running room for my crown. You want 4mm from top of tissue to implant platform. This is the cornerstone of ZBLC.

          • @drhannahtsai ZBLC stands for zero bone loss concept – its a term popularized by a Book by a Lithuanian (I think) doctor by that name.

          • 1-2mm subcrestal should be fine for the implants you got Hannah 🙂 What I like to do is to adjust bone at time of implant placement simply so that you have decent clearance for the future impression parts and abutment.

            Once you have used all the implant drills and right before you place your implant, you can just use a small round bur to create a very slight slope at the crestal part of your osteotomy. I’m talkin super slight. like just in the first mm.

            You can also do this adjustment after the implant is placed, but at that point you risk hitting the platform. If I adjust when the implant is in place, i place the cover screw.

            • I agree that as long as you have a good emergence (which like Ivan said you can do when you place, hr you may not need it) you should be golden!
