Tough tough overall case. Patient is a 15yo female with aggressive perio. Cases like this take careful planning and consideration because youre trying to plan for the next 60+ years.

I’m partnering with a prosthodontist for this case and we have a tentative tx plan but I’d love to hear outside opinions!

      • I simply refuse to believe they are 15 🙁 that’s so sad!

      • That is so tough! Definitely not your run-of-the-mill case! I would love for you to share some of the options we should be considering here.

Specifically your thoughts on implants in a person so young!

        • Absolutely!

For a female we are trying to wait until 18yo before any implant placement. If you place them too early while they are still growing, the implant can be in the completely wrong position once they are done growing.

In a case like this with a young patient with severe severe perio our main goal is to stabilize her. This means getting rid of the hopeless teeth (3, 8, 9, 14) and trying to maintain the other teeth with perio tx (including surgical) until she is older. She will likely be an eventual hybrid patient given the severe bone loss but the further we can get her along the better.

          • Wow, that’s a tough one. My first instinct is to just maintain as long as possible to allow for growth. Hopefully she can be stabilized long enough to be able to get some implants in there. I had a patient come in who had all her teeth removed at 16 and had been in dentures for 40 years, unfortunately no bone left for all on x. 😞
