    Hello Docs,

Looking for some guidance on restoring this case.

The patient had these Astra EV implants placed by a periodontist. The plan with the then restorative dentist was CONUS attachments.

The patient left the restorative dentist and came to us. He no longer wants to do CONUS or a bar overdenture due to finances.

    I was planning to…

  • CUSTOM HEALING ABUTMENTS

Hey folks! Hope everyone had a wonderful thanksgiving!

I went down a custom healing abutment rabbit hole yesterday and wound up with a few lingering questions.

The protocol for immediate implants makes total sense.. but how can we make custom healing abutments for delayed implant placement?? Does anyone do this and have…

    • Hey @RYAN Thanks for sharing this man. You got me going down the rabbit hole as well.

I’ve periodically make them to use on the day of implant placement. Not so frequently as a stage 2 procedure.

      A couple things:

The product you shared led me to several instructional videos that tried to teach me to use some free new software. The truth is, i…

#25 was lost in a car accident and replaced with this absolute beast of an implant @ 18yo

Last month, the patient presented with #26 broken off at the gumline, so I extracted it with the intent to remove the existing crown on #25 and replace it with a new custom abutment, crown… Read more

    • Not a fan of cantilever because it overloads the base implant to carry more weight and pressure than it was intended to, so that’s why I always avoid such a treatment plan. Now I do understand that we always have to work with what we are presented with and the problem with the case here is that the implant was supposed to be at a different angle… Read more

    • Definitely a can of worms. Given his situation it sounds like we want to make the best with what we can it get him healthier and ready for larger more important treatment. I would most likely do something similar. I would cut the bar and try to use the two anterior implants and get what function out of them I can and reline and/or do a…

      • Super interesting case Ryan. Do you have any additional pictures?

Helping patients in this situation can be tough.

The back two implants are obviously goners. Your plan to remove them is sound.

You might consider removing the bar and sectioning it to remove the back two implants.

The anterior portion can stay intact. You can re-insert the…
