Question, after doing a full arch for an overdenture, (meaning placing 4 implants with cover screw, and primary closure), how long do you wait to remove sutures. I used PTFE. Is it recommended 3 weeks, 6 weeks, 2 weeks? Appreciate the help.

No official timeline it’s what you’re most comfortable with. I’ve done it at two weeks in the past, but I prefer three weeks as I see more consistent healing over a wide variety of patients so I always do three weeks. Six weeks is a really long time with those sutures in there And overkill in my opinion

I’d say any of those options except 6 weeks. PTFE stays super clean, I really like suturing with that!

I wonder if that’s right and if not what I should do because I figured that Neodent is not compatible with Zest and I had to go with Novalok.

Zest locators are made for Neodent implants and you would use the same zest locator driver to torque them to your implants. You always can call them in the future as well and they can confirms those questions as well as their website

Yup, i think that Zest is basically the go-to solution for denture snap-in abutments. You can’t go wrong with them.
I also like to use DESS abutments for denture snap-ins. Both work great.

I’m not sure just from those slices but more reward and less risk in other locations. Around the mandibular midline you have your superior and/or inferior foramina relative to the genial tubercle/mental spine which can very in size greatly from pt. to pt. and often times very apical where no teeth have been before you can have more variability…

100% in agreement with @restoredsmiles While the anterior mandible is easy pickings for implants, it is very important to pay attention to things like this in the x rays.
It looks like if you drill into that spot you may get a bleeder that might be difficult to manage. I would also recommend opting for modifying your plan to stay away from that finding.

at what torque value is considered “over torqueing”?

It depends on the implant system, as well as the diameter of the implant. Typically implants can be torqued at very, very high torque values and I am not one that believes in Osteo pressure necrosis. For an mandibular 2 implant overdenture what do you think is bare minimum width for a 3.2mm implant? I know we try for 1.5 mm but I also know Ivan said we could get away with 1mm for lateral incisors. Is this the case for an overdenture in this area too?

You typically want 2mm all the way around your implant b/c implants require much more bone and biology around them than teeth do. If less than try to favor the buccal bone more (i.e. more bone to buccal of implant than lingual). 1mm only all the way around will not be successful consistently so try to have more bone especially for overdentures…

One tool to keep in mind is mini implants. People love to hate them–but they are great for situations in which you have minimal bone and it may not be feasible or practical to undergo significant grafting.
for example, 80 year old woman that just wants her dentures to not move around. I wouldnt recommend a hip graft. Instead i would use a couple mini’s.

Very important when doing any type of bone augmentation that you ask yourself where is my blood supply coming from and how can I get greater access of cells to my bone graft material.
You always want to make sure that you decorticate the buccal plate to help stimulate bleeding and allow greater access to cells to feed your bone graft…

Possible Implant Direct, or replace implant from Nobel. I would also recommend sending that information to your lab that sees lots and lots of different implants and they also could probably give you an idea what it is. I also find the website whatimplantisthat.com can be helpful as well
