Being grafted again alone does not necessarily decrease success rate. However, if the patient is showing a consistent delayed healing and/or poor reaction to treatment and there might be some underlying issue that is affecting overall healing, and that would be an account when doing further treatment.

For implants I place for overdentures I prefer to place them more subcrestal than other implants. Typically 2 to 3 mm below the crest of the bone. Over the years I just find I have better overall crystal bone and less issues and more compromised patients as bacteria tend to leak down those locators more frequent overtime and this helps…

Here are some screenshots of the CT for the case I am planning. Should I be concerned about the circular radiolucent area below 22 on page 2? When I traced it back I only saw it in a few segments. I remember Ivan saying there could be anterior branches of the IAN or vasculature in this region.

True about silk sutures, I will have the office purchase some monofilament sutures for next time. Thank you for the reassurance, I was ready to throw in the towel and give up on it especially seeing these results 2 weeks post op. Not sure if I overheated the bone to be honest, constant irrigation was used during the osteotomy. I did put the pt on CHX, will see them back in about a week to reevaluate. Thank you for your help!

Based off the limited info I see here it doesn’t appear the pt. Has sufficient bone for either of those procedures for long term success of the implants. Ridge augmentations possibly to improve success and have sufficient bone and biology for the implants or possible another route such as Zygo’s and/or ptero implants. Either way…

Very easy to remove if needed by just reversing it out with your torque wrench and implant driver. However, I agree with Adis that it’s a bit too soon to throw the towel in. First, I wouldn’t ever use silk sutures and they do collect a ton of plaque, bacteria, and food and are not very hygienic. Monofilament sutures will keep site cleaner.… Read more

I wouldn’t be too concerned out the anterior cantilever effect and it has great support with all splinted implants. However, looks like he has plenty of lip suppor so you could splint the difference and it’s a little bit easier for the lab to make a very cleanable prosthesis and it convex with not concavities. Any chance you overheated the bone during osteotomy prep? You could put patient on CHX and wait to take an xray at 1 month post placement to check on the bone, all may not be lost yet. If it looks worse, you can still remove and replace immediate or graft and delayed.
