Long story short. I inherited this.
But the previous crown had a plastic abutment under it. Did not get a pic if that. Its a Zimmer TVS implant.
The facial margin at the crown/PEEK abutment? Interface was open and full of plaque, no purulence from the gjngival tissue. The platform and threads are notched from a bur.
I took the crown off today,…

Have you considered referring for some soft tissue grafting? That’s the first thing that comes to mind

Hi everyone,
I got some conflicting advices regarding socket preservation (4 walls are all preserved), and would like to clear it up here once and for all:
The material-
Salvin rep says using the fulfill plug- 80& synthetic calcium phosphate granules with 20% bovine collagen is sufficient. Bone will turnover in 6 months and become ready…

I use allograft 50/50 mix for all my socket preservation, membrane depending on if there are any Boney wall defects as you eluded to. For simple sockets 4 walls a lot of times flattening a collaplug or the like material into the shape of the socket outline, placing it over the top and a figure 8 suture will do fine. Some folks don’t even cover…

I immediately thought of you @restoredsmiles as I believe you’re teaching a grafting course in march right?
What are your thoughts on plug vs allograft.
In your opinion, Does it matter too much in a case where all 4 walls are intact?

When doing a lateral sinus lift like you were talking about yesterday, have you encountered an intrabony artery? And if you have, what is your preferred way of managing bleeding complications? Thank you!
-
Yes, very often 50% of greater you will have a PSAA (posterior superior alveolar artery) within the bony buccal wall as the window is created. Sometimes you can work around it and/or carefully through it with the help of a piezo. If cut you can apply pressure to the hard bony wall to stop bleeding or if large enough plug it up with a Perio…

When placing an immediate implant , in study club today and the dental lit states it is best to place bonegraft with a provisional. My question is does it make a difference in placing the implant first and than the bone graft? I find it easier to leave my last implant drill, place the allograft and than seat the implant. Does the dental…

Either before or after is fine, most of it just comes down to preference. As long as the gap is grafted, that is what is the most Important. If you do place the bone after often times I place a little bit at a time and then I use something like a Perio probe to gently push the bone graft material as apical as possible so I get a good…

I really like to do it this way also, I leave my last drill in the prep and pack bone around it. Then the implant compresses everything together. Although its tricky when bone ends up accidentally falling into the prep!
