It looks like there’s great interproximal Bone so that’s what you should lean into on a case like this. Here’s a full arch we did for this patient with 6 implants for a fixed hybrid. Bone reduction for these cases can be crucial especially depending on your approach and material selection. Good keratinized tissue around your multi unit abutments and/or locators are also key to long term success.

Clinical experience and research shows they have almost the same success rate as long implants. If you have the bone width, I do prefer to go wider when I go shorter if there’s enough bone and biology to allow that.

I would choose option #1. Typically if the RBH is 4mm or more I will do a Crestal sinus lift rather than a lateral sinus lift. Irregular anatomy may be easier to manipulate via the Crestal approach as well is this case.

Here is a little sinus action to get your Easter weekend off to a great start😁. Placed 6cc of bone with a 50/50 mix allograft and xenograft. We will let this cook for 6 months prior to implant placement.
